the country of origin - for foreign students)

College

a. Grade 11 Report Card – for currently enrolled in Grade 12
Grade 12 Report Card – for Senior High School graduates
Transcript of Records – for transferees / second college degree
applicants
b. Birth Certificate (PSA/NSO – for Filipinos / Birth Certificate issued by
the country of origin - for foreign students)
c. Marriage Certificate – for females who prefer to use their married names d. 2X2 picture (white background)
e. For foreign students only

School of Law

a. Bachelor’s degree Transcript of Records
